The Guardian Model™
The Guardian Model™
When the system is fragmented, families carry the weight.
The Guardian Model™ was created to change that. It brings legal systems, mental-health providers, hospitals, families, and community supports into one coordinated structure built around the whole person.
THE PROBLEM IS NOT EFFORT. IT IS STRUCTURE.
Too often, every provider completes one part of the job while no one takes responsibility for how the pieces work together. Families are left repeating the same information, tracking every promise, chasing updates, and trying to prevent the next crisis.
The Guardian Model™ establishes one plan, clear roles, connected communication, and meaningful accountability. Everyone knows what is happening, who is responsible, and what must happen next.
THE SEVEN C’S OF COORDINATED CARE
Client-centered — The person’s needs, dignity, goals, and voice remain at the center.
Comprehensive — The plan considers the whole situation, not one isolated diagnosis or event.
Collaborative — Families, professionals, and community supports work from the same understanding.
Communicative — Important information reaches the people who need it when they need it.
Consistent — Support does not disappear when providers, settings, or circumstances change.
Committed — Responsibility continues beyond a referral, meeting, or discharge.
Coordinated — Every decision connects to the larger plan.
WHAT CHANGES
Families stop serving as the only bridge between disconnected systems. Providers work with clearer information. Responsibilities are visible. Follow-through can be measured. Gaps are identified before they become emergencies.
The model has been applied in real-world settings associated with reductions in psychiatric hospitalizations, crisis-level care, emergency-room use, and law-enforcement involvement.
This is not about asking families to carry more. It is about building a structure strong enough to carry the work with them.
Clear roles. Connected decisions. Shared responsibility. A plan that holds.
